Our standard practice forms for new patients are now available for downloading. 

New patients can: (1) DOWNLOAD these forms (Patient Face Sheet & Consent Form, Psychosocial Case History, Release of Information, Verification of Receipt of HIPPA Regulations, HIPPA Notification of Privacy Practices),  (2) PRINT them out in color, (3) COMPLETE them at your own convenience, and (4) BRING them with you already filled out to your first appointment.

Forms are either in Microsoft Word or Microsoft Publisher format; all brochures are in Microsoft Publisher format. If you do not have Microsoft Word or Microsoft Publisher, simply go to www.openoffice.org and download the free software package. Open Office programs are compatible with Microsoft Word and Microsoft Publisher files.
